Abstract
Introduction: The clinicians' decision regarding immediate loading is largely determined by the assessment of initial stability of the dental implant. Computed tomographic assessment of recipient site bone quality, insertion resistance torque and post-insertion resonance frequency are currently the only objective methods of measurement establishing initial stability.
Purpose: A study was designed to measure resonance frequency of root form dental implants placed in cadaver jaws. The resonance frequency values were compared to previous subjective and objective data reported in prior studies in the series.
Materials and Methods: Resonance frequency measurements were secured using the Osstell ™ mentor unit on 4.0 X 10.0 mm, Mark III implants placed into a wide variety of edentulous cadaver jaw specimens. Two examiners recorded triplicate measurements on each of 41 implants. These values were compared to Lekholm and Zarb (L & Z) subjective bone quality scores, Misch subjective drilling resistance scores, and extent (by thirds) of implant insertion at 40 Ncm. Comparisons were made to the objective measures of computed tomographic Hounsfield Units (HU) and insertion resistance torque recordings.
Results: No convincing relationship between resonance frequency analysis and the battery of subjective and objective parameters was found. Pearson correlations of 0.5 were produced when RFA was compared to computed tomographic measures. A correlation of 0.4 was calculated when RFA was paired with insertion resistance torque recordings.
Discussion: Lack of reliability in relating RFA to other subjective and objective methods of initial stability evaluation may be a consequence of methodology and cadaver specimen analysis. Testing RFA in the clinical setting may prove differently.
Conclusion: RFA does not appear to independently measure initial stability of the root form dental implants in cadaver specimens.
