Abstract
An in vitro performance evaluation and comparison of a patient individualized appliance (Elan, Ormco Corporation, Glendora, California) and a Roth prescription appliance ("A"- Company, San Diego, California) was made using the Ideal Tooth Relationship Index (ITRI).
The sample consisted of 19 duplicated sets of models for a total sample of 38. For each pair of models one set was bonded with an Elan appliance and a duplicated set was bonded with a Roth appliance. Bracket placement followed the manufacturers recommendations. The bracketed teeth from these models were sectioned out individually and the appliances allowed to fully express themselves on full size steel arch wires (.018 x .025 in a .018 slot). The teeth from each model, tied into the arch wires, were then occluded to a best fit occlusion and evaluated. Appliance integrated individualized arch forms, provided from the manufacturer, were used with the Elan appliance brackets. "A"-Company Tru-Arch arch wires were used, as designed, with the Roth prescription appliances. No subjective manipulation to bracket placement or arch form was allowed.
ITRI performance scores revealed statistically significant differences in total score, anterior inter-arch, anterior total, and posterior lingual arch segments, with the Elan appliance performing superiorly. No significant performance differences were found in the intra-arch, posterior buccal, or posterior total arch segments.
In general, neither appliance performed sufficiently to be considered a true "straight-wire" appliance. Bracket placement appeared to be the main limiting factor in overall performance for both appliance systems. In vivo investigations will be necessary to determine clinical significance of appliance differences including total duration and quality of treatment.
