Abstract
Buccal arch positioners (BAP) were constructed for four orthodontic patients banded with edgewise brackets, which were clinically observed during the finishing stages of treatment. At 2-4 week intervals data were collected on oral hygiene, tissue tolerance, magnitude and duration of soreness, and ease of appliance insertion and adjustments.
During the construction of the BAP, an ideal set up was found to be necessary for construction of the flexible components. It had the final location of each tooth visible so the investigator could work towards that goal.
Heat treatment of the rigid component was done to remove the cold working stress placed on the wire during formation and to remove the tendency to return to its original form. Also placement of the rigid component in patients with a shallow vestibule or restricting muscle attachments was more critical.
Clinical observation showed that oral hygiene was maintained well. Soft tissue tolerance and intraoral adjustments of the appliance were good. The magnitude of soreness after each appointment was mild and the mean duration was 1.4 days.
It was illustrated that when anterior space closure or torquing was done there was a need for an auxiliary force or the buccal segments would move mesially. This occurred because the sum of the reciprocal forces of the flexible components to the rigid component was in a mesial direction.
The BAP was able to produce tooth movements by using standard edgewise brackets without the critical positioning of the bracket on each tooth as is necessary in conventional edgewise techniques.
