Abstract

The purpose of this study was to compare the two-dimensional cephalometric and the three-dimensional tomographic radiographic methods in their ability to assess anatomic measurements while using the Onplant anchoring device in anterior retraction cases.

The Onplant was surgically placed beneath the periosteum of the hard palate. After osseointegration the Onplant was then uncovered and attached to the first molars by means of a transpalatal bar. Conventional orthodontic treatment was then used to retract the anterior teeth after 1st premolars were extracted.

To evaluate the movement of the Onplant, a three-dimensional measuring tool was needed. Seven different measurements were made on the NewTom and then made with traditional methods (study models, frontal cephalogram, lateral cephalogram). The position of the Onplant relative to three neurotropic centers (incisive canal, left and right foramen rotundum) was also measured. Each measurement was made on four subjects. All measurements were repeated three times.

The NewTom has demonstrated a measurement capacity that concurs with traditional methods of orthodontic evaluation. The NewTom agrees with the study models more frequently and accurately than the frontal and lateral cephalometric measurements. This encourages the use of NewTom images instead of cephalometric images in order to evaluate treatment outcomes when using the Onplant.

The position of the Onplant relative to neurotropic centers of the head as measured on the NewTom image has been shown to be reproducible and reliable. With this information the NewTom can be used to show movement of the Onplant in any dimension relative to neurotropic centers of the head throughout treatment.

The first stage of this evaluation of the Onplant and the results obtained obviate the need to evaluate the anchorage capacity of the Onplant and the dental movements achieved at the end of treatment by means of NewTom imaging.
