Abstract
This study was conducted to clinically evaluate cuspid and anterior retraction in first bicuspid extraction cases using the Light Progressive Technique. More specifically interest was centered on rate of cuspid and anterior retraction, anchorage preservation or control, difficulty of appliance adjustments, time involved in adjustments, frequency of adjustments, total retraction time, patient discomfort, root resorption, as well as a complete cephalometric appraisal. Seven patients were treated for ten months. The appliance was activated at three week intervals, and progress recorded monthly.
It was shown that this technique maintains molar anchorage during rapid cuspid retraction without relying on patient cooperation for appliance activation or for extraoral molar stabilization. However, this technique was found to moderately difficult and painful. Maxillary cuspid retraction averaged 1.14 mm/month with a range of .79 to 1.61 mm/month. Anchorage lost averaged .26 mm/month with a range of -.17 to .67 mm/month. Mandibular cuspid retraction averaged 1.10 mm/month with a range of .75 to 1.94 mm/month and molar movement averaged 0 mm/month but varied from -.90 to .56 mm/month.
Intraoral radiographs revealed cuspids tipped an average of 21.6° during retraction. Cephalometric analysis showed maxillary incisor retraction of 4.5 mm and tipping of 11.5° relative to NA. Mandibular incisors were retracted 2.9 mm and tipped 7.1° relative to NB. The mandibular plane increased approximately 1.5° during the treatment period.
Incisor retraction was incomplete and data insufficient to project results.
