Abstract
The present pilot study is a histological evaluation of mandibular condyles taken from minipigs that underwent either traditional mandibular advancement (bilateral sagittal split osteotomy), or a distraction osteogenesis procedure to advance the mandible. One pig received a bilateral sagittal split osteotomy with an immediate mandibular advancement of 14 mm, while another pig underwent distraction osteogenesis consisting of a gradual advancement of 2 mm per day to advance the mandible the same distance. The control animal received no surgery.
In the present study, the mandibular condyles of the three pigs (2 control, 2 bilateral sagittal split osteotomy, and 2 distraction osteogenesis) were prepared for histological evaluation. The condyles were sectioned parasagitally, perpendicular to the occlusal plane. These sections were then prepared using a nondecalcified method to produce histological for evaluation.
The bone density was calculated for a strip of bone extending 0.5 mm from the articular surface using a point counting method. Each measurement was performed three times. The bone density percentage was found to be significantly less for the bilateral sagittal split osteotomy pig than for either the distraction osteogenesis pig, or control pig.
There was no significant difference between the distraction osteogenesis pig and the control pig.
Qualitative analysis was done to evaluate any other hard or soft tissue changes following the advancement procedures. The condyles of the bilateral sagittal split osteotomy pig showed more degenerative changes than both the distraction osteogenesis pig and the control pig. The distraction osteogenesis pig also showed some degenerative changes when compared to the control pig.
This study suggests the need for future studies with larger sample sizes and more quantitative results. These larger studies will likely confirm these initial findings.
