Abstract

Objectives: This study examined the agreement of summary description and treatment plan elements derived from cone beam computed tomography (CBCT) and 3-Tesla (3T) magnetic resonance imaging (MRI) scans.

Methods: This retrospective study involved thirteen human subjects, all of whom had one CBCT scan (NewTom 5G, AFP Imaging, USA) and one MRI scan (Siemens Medical Solutions, DE) of the whole head performed within a two-week period. CBCT images were taken with a field of view of 18x16 inches and resolution of 0.3x0.3x0.3 mm, over an exposure time of 5.4 seconds. MR images were acquired using a 3.0T imaging system with a 12-channel head array coil and resolution of 1.0x1.0x1.0 mm, over an exposure time of 4-5 minutes. Patients were imaged in a supine position for both CBCT and MRI scans. Osirix MD software (V.12.0.3) was used for landmark identification of the resultant DICOM images and QuickCeph Studio (V 5.0.1) for cephalometric analysis (Steiner and Ricketts). For research timepoint 1 (RT1) half the patients were chosen at random to use CBCT for landmark identification, cephalometric analysis and fill out summary description (SD) and treatment plan elements (TPE). The resulting patients were analyzed using MRI. After a 2-week washout period, in research timepoint 2 (RT2) the measurements were repeated with the opposing image modality from what was used at RT1. Statistical analysis was performed with cohen’s kappa and intraclass correlation coefficient (ICC) to evaluate intra-rater agreement when using CBCT or MRI.

Results: Excellent agreement was observed between CBCT and MRI between RT1 and RT2 in all summary description and treatment plan elements with exception of mandibular position (p-value = 0.07). Average Kappa = 0.75, ICC values remained above 0.5 with exception of maxillary position (ICC = 0.40) and mandibular position (ICC = 0.36).
