Abstract

Treatment of the intrabony or intraosseous periodontal defect has rendered much attention over the last few decades. Numerous approaches have been advocated; however, overall comparisons between treatments do not exist. This review summarizes and discusses outcomes of therapeutic modalities in the intrabony periodontal lesion from controlled clinical studies. The modalities reviewed include scaling and root planning, gingival flap surgery, gingival flap surgery including wound closure with interdental denudation, root-biomodification, placement of autologous, allogenic, or xenogeneic bone grafts, placement of alloplastic grafts, and placement of tissue guiding barrier membranes. In addition histologic observations of healing in the intrabony periodontal lesion are reviewed.

The effect of citric acid conditioning of the root surface in conjunction with gingival flap surgery including barrier membranes (expanded polytetrafluoroethylene) was clinically evaluated in intrabony periodontal lesions in 22 patients. Control treatment included gingival flap surgery with barrier membranes alone. Fifteen lesions were treated with the experimental and 16 with the control protocol. Healing was evaluated 12 months after surgery. Initial probing depths approximated 6.9 mm and defect depths measured during surgery exceeded 4 mm. The patients exhibited good oral hygiene over the study interval as substantiated by low plaque and bleeding scores. Acid conditioning of the root surface did not enhance periodontal healing in this study. A similar amount of defect resolution was observed following either treatment protocol. Probing reduction generally amounted to 1.8 mm, gain of attachment level to 0.8 mm, and defect bone fill to 1.2 mm. The results are compared and contrasted to those from clinical studies including barrier membrane techniques or acid conditioning of the root surface.
